• Eq. (C.17): l(l+1)-4 should be l(l+1)-2.
• Eq. (H.23): Remove \gamma_{\rm E} from {3424\gamma_{\rm E} \over 21} \log(16x).
• Eqs. (1.17) and (1.20): Remove minus sign in front of 16 \pi. (See Appendix F, which gives correct equations.)
• Eq. (1.230): in the last term of the first line, add \kappa_{\rm I}^{-1}
• Eq. (2.218): the first term in the rhs; A_{ac} --> A_{ab}
• Eq. (1.175): minus sign is necessary in front of "ln h".
